Thank you for considering Montessori Children’s House for your little one! We are very excited to welcome you to our school. In order to secure a spot for your child, the following items are needed:
Child’s Virginia School Health Entrance Form
This documents the child’s physical exam or last well visit. May be turned in up to 30 days after your child begins school, according to Virginia regulations. A blank copy is available under Forms.
Registration fee of $100
Registration fees are nonrefundable. Please make checks out to MCH.
One complete change of clothes
Please place in a one gallon Ziplock bag labeled with your child's name. Don't forget socks and underwear! Your child will change into these in case of a spill or bathroom accident.
Enrollment paperwork
Please complete all questions or mark “N/A.” Blank answers will not be accepted by Virginia Department of Social Services, who oversee all licensed childcare centers. Please see the Forms tab.
Child’s original birth certificate or passport
We are required to look at the original for age verification.
Child’s immunization
records
Must have prior to child’s attendance at MCH. May use physician’s form, or Virginia School Health Entrance Form.

Please bring the following for your first day of school:
Preschoolers who stay past 12:45 will need a nap mat
We recommend roll-up nap mats with an attached pillow and a blanket that covers the child. Amazon has great prices.
Ziploc “big bag” size L or XL to store the nap mat, if applicable
Please label with your child’s name.
Tote bag for kindergarten and elementary students only
Please no backpacks. Preschoolers do not need a bag.
Lunch box and water bottle
Please label both with your child’s name. To help your child keep track of his/her water bottle, we recommend a lunch box that is big enough to hold the water bottle, or has a pocket for the water bottle. Please include an ice pack with lunch if the food must remain cool.
